Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Active, unrestricted U.S. pharmacist license.
• Experience in hospital, ambulatory, community, or clinical pharmacy.

Responsibilities

• Review prior authorization requests for clinical appropriateness and plan criteria.
• Apply evidence-based guidelines and formulary rules to approve or deny requests.
• Communicate determinations to providers and members with professional documentation.
• Collaborate with medical directors and clinical teams on complex cases.
• Document all decisions in CMS, state, and health-plan compliant manner.
• Support process improvements to reduce turnaround times and improve member experience.
