Job summary

PACS seeks a Reserved Bed Post-Acute Care Manager to serve as the liaison between hospital partners and our post-acute centers. You will coordinate referrals, monitor admissions, and ensure compliant, efficient patient placement across a growing program.

The role emphasizes building strong hospital relationships, tracking KPIs, and reporting outcomes while traveling to partner sites as needed. Remote work supported with travel as required.

Qualifications

• Must possess a high school diploma or GED.
• Bachelor's degree in healthcare or related field preferred.
• Two (2) years of Admissions experience preferred.
• LVN or RN license required.

Responsibilities

• Educate hospital case managers on the Reserved Bed Program, referral criteria, and admission processes to facilitate efficient transitions of eligible patients into PACS post-acute care centers.
• Receive Reserved Bed referrals from hospital case managers and review patient records to determine eligibility in accordance with established admission criteria and stop-list guidelines.
• Manage the referral process by distributing referrals to PACS facilities, securing placement, and communicating outcomes to hospital case managers; escalate unplaced referrals as needed.
• Assess patient financial and insurance resources (Medicare, Medicaid, Managed Care, HMOs, and commercial plans) to support placement decisions.
• Coordinate patient transfers to PACS facilities by ensuring orders, documentation, meds, equipment, and services are arranged prior to admission.
• Participate in daily/weekly meetings to strengthen partnerships and ensure program success.
• Maintain accurate documentation of referrals, responses, admissions, denials, and placements within tracking systems.
