Southern Illinois Healthcare is seeking a Registered Nurse or LPN licensed in Illinois to join our Population Health team in a fully remote capacity. The role emphasizes patient advocacy, care coordination, and coaching patients toward self-management, with a focus on chronic disease management.

Responsibilities include driving care coordination, maintaining caseloads, and developing individualized care plans in collaboration with the care team.

Qualifications

• Minimum of three-year experience using Electronic Medical Record systems (EPIC preferred).
• Background in ambulatory and/or inpatient acute care nursing.
• Ability to effectively communicate with physicians, care team members, and caregivers.
• Clinical acumen to support disease progression and interventions.
• Basic knowledge using web-based meetings and MS Office tools (Excel/Word/PowerPoint).
• Remote work experience preferred.

Responsabilités

• Drive care coordination for patients with chronic diseases.
• Manage multiple caseloads and deadlines independently.
• Develop and maintain individualized care plans for a patient cohort.
• Apply clinical coaching methods to promote patient engagement and self-management.
• Coordinate care with Care Team members and maintain training/certification.
