Job summary

Southern Illinois Healthcare is seeking a Registered Nurse or Licensed Practical Nurse for a fully remote Population Health role in Illinois. You will advocate for patients, coach healthy lifestyles, and partner with care teams to improve quality measures.

The position requires an Associate Degree in Nursing and an active IL LPN license (RN preferred), plus at least three years of EMR experience (EPIC preferred). Remote work experience and strong communication are desired.

Qualifications

Associates degree in nursing required.
Current LPN nursing license, pursuing RN; RN strongly preferred.
Minimum of three-year experience using Electronic Medical Record systems (EPIC preferred).
Strong background in ambulatory and/or inpatient acute care nursing.
Ability to communicate effectively with physicians and care teams.

Responsibilities

Uses nursing knowledge to drive care coordination for patients with chronic diseases.
Maintains multiple caseloads and deadlines independently and on time.
Develops individualized care plans and goals for patients within cohorts.
Applies clinical coaching to promote patient engagement and self-management.
Coordinates with care team members to optimize care.
Conducts concurrent medical record reviews for care gap reporting.
Assists with orders for laboratory and diagnostic testing per guidelines.
Partners with community programs to promote Population Health.
