Job summary

Family Health Centers at NYU Langone is seeking a Peer Counselor to lead or co-lead WRAP/WSM groups, promote the COD program, and enroll patients into the Entry program. The role includes connecting patients with Case Management, Psychiatry, or Counseling sessions and performing outreach by telephone or reminders.

The position may be performed onsite or remotely from home or a designated location. NYS Peer Specialist certification or the ability to obtain within one year of hire is required,

Qualifications

• Must hold a NYS certification as a Peer Specialist or be able to obtain certification within one year of hire.
• Demonstrated interpersonal communications skills and the ability to empathize with participants of mental health services.
• Active participation in mental health self-help activities, peer support, or similar experiences.
• Willingness to work outside of office setting in the community and participants' homes.

Responsibilities

• Educate patients about the importance of medical and mental health care and attending all program appointments.
• Educate patients about the COD program and services offered, and answer questions about the program.
• Outbound outreach by telephone to check on engagement and motivate attendance.
• Follow up on non-attendance and upcoming appointments to re-engage patients.
• Lead or co-lead WRAP and/or Wellness Self Management groups with new patients after each cycle.
• Assist patients with information about community resources and link to Case Manager.
