Job summary

Trillium Health Resources is seeking a Peer Support Specialist to join the STAR team. The role supports adults exiting incarceration, providing care coordination before and after release to ensure smooth transitions into community care management.

You will educate members and guardians about navigating behavioral health, housing, employment, and community services, coordinating with the care team and community resources.

Qualifications

• High School Diploma or GED required.
• 1 year lived experience with a behavioral health condition and/or substance use.
• North Carolina Peer Support Specialist Certification.
• Valid driver’s license.
• Must reside within 75 miles of Trillium’s Mid State Region and be able to travel within catchment.

Responsibilities

• Connect recipients to needed treatment and support.
• Meet justice-involved members in correctional facilities and in the community.
• Educate members about housing, services, and resources.
• Coordinate transition planning with care team for optimal outcomes.
• Assist with linkages including school, employment, housing, and insurance access.
