Job summary

Saint Luke's Crittenton Children's Center is seeking a remote In Home Services Case Manager to support a Pediatric Behavior Health Team in the Kansas City area. You will manage a caseload of children aged 4–18, collaborating with Tri-County Mental Health and providing care in the home and community.

Responsibilities include treatment planning with a Master level clinician, linking families to resources, coordinating care with schools and providers, and offering 24/7 on-call support.

Qualifications

• Bachelor's degree required.
• Minimum 1 year of applicable experience.
• Driver's license required or acceptable as per locale.

Responsibilities

• Manage a caseload of pediatric patients (ages 4–18) with a remote home-based model.
• Develop and implement treatment plans with a Master level clinician.
• Link families to community resources and coordinate care with partners.
• Provide 24/7 on-call availability.
• Serve Clay, Ray and Platte counties in the community setting.
