Responsibilities/Job Function

  1. Responsible for monitoring vital signs remotely and calling the patient to assess need for intervention by their healthcare provider or whether further monitoring is appropriate.
  2. Building relationships with patients for engagement, support and communication.
  3. Remote patient onboarding, education for monitoring devices, mobile app, and platform.
  4. Remotely monitoring enrolled patients through daily review of electronic patient data (adherence, physiological data, patient messages, etc.).
  5. Providing education and other tools to support improved patient outcomes.
  6. Communication between the provider and other members of the care team, as needed.
  7. Initiates phone contact with eligible Medicare patients in attempt to enroll them in CCM/RPM/RTM programs on behalf of physician practice.
  8. Completely and accurately educates eligible patients on the benefits of CCM/RPM/RTM programs.
  9. Obtains consent from patients electing to opt-in to the programs.
  10. Ensure a seamless, complete and effective enrollment process for all prospective patients into the care monitoring programs.
  11. Maintain access to, and security of, patient data and information.
  12. Troubleshoot devices and report malfunctions.
  13. Answer routine inquiries and relay information and messages to and from appropriate health professionals.
  14. Attend customer meetings.
  15. Will be required to interact with customers and patients in multiple time zones.
  16. Travel up to 30% as needed.
  17. Other duties as assigned.

Personal Skills/Attributes/Qualifications

  1. Certified Medical Assistants/LPN/CNA - certification required.
  2. 2- 5 years of related professional or healthcare medical experience.
  3. Spanish language speaking and writing skills are highly desirable.
  4. Strong analytical and problem-solving skills.
  5. Enjoys fast paced work environment with a positive attitude.
  6. Self-motivation and the ability to work independently and with teams.
  7. Proficient in the use of Word, Excel, and PowerPoint.
  8. Excellent oral and written communication skills.
  9. Proficient in managing multiple tasks at the same time.
  10. Tech savvy.
  11. Experience in using EHR systems.

Pay: $17.00 - $20.00 per hour
