We are currently searching for a Remote Patient Care Specialist. This is a fully remote role, you must reside in the United States.

What you’ll do:

The Remote Patient Care Specialist works to enhance patient satisfaction and clinician efficiency through a collaborative approach to patient care. The position is 100% remote and supports the local care team by conducting pre-visit planning and patient care support to ensure all needed information is available at the patient’s appointment. Care opportunities are identified, and patient outreach and scheduling conducted when needed.

General Duties and Responsibilities:

  • Responsible for conducting pre-visit planning, outreach, scheduling and verification activities for preventive services, care gaps, immunizations, medication reconciliation and Annual Wellness Visits (traditional Medicare, Medicare Advantage and commercial).
  • Conducts chart reviews and ensures all diagnostic results, specialists reports, medication refills and other clinical requests/orders are complete and available for the provider; when necessary, calls to obtain results; notes any missed diagnostic tests or specialist referrals.
  • Review available insurance payor reports to identify gaps in care, medication adherence opportunities and documents care opportunities in the medical record as a pre-visit note.
  • Uses sound clinical judgement to ensure medical record is fully prepared for the physician at the time of the patient’s visit.
  • As needed, pre-registers patients including collecting and/or updating patient demographic and billing information, verifying insurance coverage, etc.
  • Maintain a current and working knowledge of HEDIS quality measure specifications, care requirements and eligibility specifications associated with different types of wellness visits.
  • Strong collaboration with the practices through effective communication, compassion, and appropriate judgement in patient care.
  • Excellent communication and advanced organizational skills are required.
  • Informs patient of any/all necessary pre-procedure preparations, as directed by the practice or per written practice protocols.
  • Works closely and coordinates with hospital case managers to schedule discharged patients for a post-discharge appointment, conduct transitional care call within 48 hours post discharge, ensures discharge summary is available and documented in the medical record.

What you’ll need:

  • Associate’s degree or Medical Assistant formal training preferred.
  • 5+ years’ experience required preferably in a physician office/ambulatory setting.
  • Certified Medical Assistant.
  • Strong organizational skills.
  • Ability to work in a team environment.
  • Computer Skills: Excel, Word, Outlook, Medical Billing Software Systems.
  • Excellent communication skills and ability to work in a team environment.

Pay Range: $17 – 20/hour. The final agreed upon compensation is based on individual education, qualifications and experience.
