DirectShifts is seeking a multi-state licensed Primary Care Physician (MD or DO) to join our team in a part-time, remote capacity. The ideal candidate will screen patients for eligibility to participate in support programs tailored to individuals with chronic illnesses.

Qualifications

• Licensed Primary Care Physician (MD or DO) in good standing.
• Actively enrolled with Medicare.
• Familiarity with chronic disease management and care coordination.
• Experience reviewing medical records and performing eligibility screenings.

Responsibilities

• Screen and assess patients for eligibility for programs and care coordination.
• Review patient charts for compliance and appropriateness for program inclusion.
• Collaborate with care team to support chronic-condition patient profiles.
