Job summary

Ennoble Care is seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to join our remote palliative care team. The LPN will support patients with serious illnesses by providing ongoing clinical coordination, symptom management, and caregiver education via telephone and virtual visits.

This role offers remote work flexibility and requires strong communication, empathy, and the ability to work independently while collaborating with an interdisciplinary care team.

Qualifications

• Current, unrestricted LPN license in the applicable state(s).
• Graduate of an accredited practical nursing program.
• Experience in palliative care, hospice, home health, or related setting preferred.
• Strong clinical assessment, communication, organization, and documentation skills.

Responsibilities

• Manage scheduled telephone and/or virtual patient check-ins under supervision.
• Conduct high-acuity patient chart reviews for hospice clinical appropriateness.
• Monitor and document patient-reported symptoms, changes in condition.
• Provide education to patients and caregivers on medications and symptom management.
• Escalate urgent concerns to the RN/provider or interdisciplinary team.
• Maintain accurate documentation in the EMR and participate in team meetings.
