Health Quality Innovators is seeking an experienced Outpatient Quality Improvement Consultant to support outpatient practices across the Southeast through remote collaboration and occasional onsite visits.

The ideal candidate will combine clinical expertise with data-driven quality improvement, build strong relationships with practice leaders, and coach teams to implement evidence-based strategies for better patient outcomes.

Qualifikationen

• Deep knowledge of HCC coding, risk adjustment, RAF score accuracy.
• Recent outpatient practice or health system experience.
• Experience with quality improvement or healthcare operations.
• Knowledge of outpatient practice quality measures and performance improvement.
• Strong presentation, facilitation, and relationship-building skills.
• Ability to work independently in a remote environment.
• Willingness to conduct occasional onsite visits within assigned states.

Aufgaben

• Provide consultation and technical assistance to outpatient clinical practices.
• Analyze quality measure and performance data.
• Conduct needs assessments and root cause analyses.
• Develop and monitor quality improvement action plans.
• Facilitate educational programs, coaching calls, and webinars.
• Build relationships with practice leadership and interdisciplinary teams.
• Support implementation of CMS quality initiatives and best practices.
• Collaborate with subject matter experts and quality improvement leaders.
