Job summary

Strategic Resilience Group seeks a senior medical coder to review outpatient EHR documentation, assign ICD-10-CM and CPT codes, and ensure accuracy with NCCI edits and coding guidelines.

You will enter and validate coding data using VA systems (encoder, VistA/CPRS, webACM) and provide daily productivity reports, supporting audits and retrospective reviews as needed.

Qualifications

• Active coding credential through AHIMA or AAPC.
• Certification options include RHIA, RHIT, CCS, CCS-P, CCA, CDIP, CPC, CPC-H.
• Completion of an accredited coding/Health Information Management program.
• Minimum 6–8 years of outpatient coding experience.
• Proficiency with ICD-10-CM, CPT, HCPCS and reimbursement methodologies.

Responsibilities

• Review complete electronic health record documentation for assigned outpatient encounters.
• Assign and validate ICD-10-CM and CPT codes; determine billable status and required documentation.
• Apply current Evaluation and Management coding guidelines and NCCI edits.
• Identify non-supporting documentation and duplicates; enter coding information in VA systems.
• Provide daily productivity and workload reporting; support coding audits and reviews.

Job description

• Review complete electronic health record documentation for assigned outpatient encounters
• Assign and validate:
  • ICD-10-CM diagnosis codes
  • CPT codes
  • Primary and specialty care
  • Ancillary services
  • Radiology and laboratory services
  • Ambulatory surgery
  • Anesthesia
  • Pathology
• Apply current Evaluation and Management coding guidelines
• Apply NCCI bundling/edit requirements
• Identify documentation that does not support code assignment
• Identify duplicate encounters or encounters created in error
• Determine when encounters are not billable and document standardized reasons using VA systems
• Perform re-reviews and corrections associated with billing edits, denials, or retrospective reviews
• Enter and validate coding information using the VA national encoder, VistA/CPRS, webACM, and other VA-approved systems
• Provide daily productivity and pending-workload reporting
• Support limited coding audits and validation reviews when requested

Required Qualifications

• Active coding credential through AHIMA or AAPC, including one of the following:
  • RHIA
  • RHIT
  • CCS
  • CCS-P
  • CCA
  • CDIP
  • CPC
  • CPC-H or recognized successor equivalent
• Completion of an accredited coding, Health Information Management, or Health Information Technology program
• Minimum 6–8 years of outpatient and/or applicable medical coding experience
• Current ICD-10 training and CEUs required to maintain certification
• Formal knowledge of:
  • Anatomy and physiology
  • Medical terminology
  • Pathology and disease processes
  • Pharmacology
  • Reimbursement methodologies
  • ICD-10-CM
  • CPT
  • HCPCS
• Strong understanding of modifiers, sequencing, bundling, NCCI edits, and documentation requirements
• Must perform all work from within the United States
• Must reside within 50 miles of a VA Medical Center
• Ability to satisfy VA background, security, PIV, privacy, and system-access requirements
