Job summary

The CORE Institute in Phoenix, AZ is seeking a licensed Physical Therapist to join our outpatient team, with remote work options and a full-time schedule. You will assess patients, develop treatment plans, and work alongside physicians to optimize care.

The role requires an active state license and, ideally, a DPT. You will supervise assistants, document care, educate patients, and uphold HIPAA standards while delivering high-quality rehabilitation services.

Qualifications

• Valid and active state license required.
• DPT preferred for clinical leadership.
• Experience in outpatient physical therapy settings.

Responsibilities

• Assess patient condition, establish goals and care plans with physician and patient/family.
• Coordinate interdisciplinary treatment and consult on PT factors affecting health.
• Document findings, treatments, and patient information for the medical record.
• Educate patient/family on appropriate care and therapies/exercises.
• Serve as resource for patients, families, and staff.
• Supervise Physical Therapy Assistants and Physical Therapy Technicians.
• Provide ongoing education to Physical Therapy staff.
• Assist the director with patient care issues.
• Maintain patient confidentiality and demonstrate current job competencies.
