Cardinal Health is hiring a Nurse Care Manager for Individualized Care, working remotely nationwide to coordinate oncology care, educate patients, and ensure seamless communication among patients, providers, and support teams.

The role requires an RN license (Wyoming or compact) and experience with care management, HIPAA compliance, and EHR documentation. Expect a 40-hour workweek with strong emphasis on patient outcomes and coordination.

Competenze

• Registered Nurse with a current, unrestricted Wyoming license or compact license including Wyoming.
• Excellent communication skills and patient education abilities.
• Experience with Microsoft Office products.

Mansioni

• Coordinate care with oncology and care management teams.
• Document encounters accurately in the EHR and maintain HIPAA compliance.
• Manage a caseload of 100-150 members and coordinate care transitions.
• Provide patient education and promote disease self-management.
• Participate in quality improvement activities and team huddles.
