Qualifications

• Bachelor's degree required.
• 8+ years of technical experience.
• 2+ years of project leadership experience.
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
• Pass TB screening as part of the role.

Responsibilities

• Provide care for patients through virtual visits
• Evaluate and treat patients according to care standards
• Monitor care quality using data and chart reviews
• Participate actively in daily care team huddles
