REMOTE Nurse Practitioner (NP) - Chronic Disease Management

We are currently partnered with a highly reputable health network who offers state-of-the-art technology and multidisciplinary clinical care team for chronic disease management, in the Indiana market. The organization increases access for patients with a fully staffed, APP led clinical care team.

Our Nurse Practitioners are responsible for appropriately triaging patients based on vitals, and managing treatment plans and medications in collaboration with the patient’s physician / provider.

Schedule : full time remote

Monday - Friday : 9a-6p CST

Compensation :

135,000 - $140,000 / year base

Qualifications :

5 years' experience as an NP treating patients with chronic diseases

T2D, hypertension, chf experience required

Active NP license

Multi-state compact license

ANCC or ANNP

Experience in remote patient monitoring technology preferred
