Elevance Health is seeking a Nurse Case Manager II (RN) for a 100% telephonic, multi-state role. The position supports members with complex and chronic needs through screening, assessment, planning, care coordination, and follow-up. A BA/BS in a health field and RN licensure across states are required.

Qualifications

• Requires BA/BS in a health related field and minimum of 5 years of clinical experience.
• Current, unrestricted RN license in applicable state(s) is required.
• Multi-state RN licensure is required for providing services in multiple states upon hire.

Responsibilities

• Conduct assessments to identify needs and develop a care management plan.
• Assist with authorizations/referrals within benefits structure.
• Coordinate internal and external resources to meet needs.
• Monitor and evaluate care management effectiveness and adjust as needed.
• Collaborate with Medical Directors and health professionals on care plans.
• Troubleshoot provider, claims or service issues and improve processes.
