Responsibilities

• Assesses member needs and develops individualized care plans.
• Coordinates authorizations/referrals within benefits structure or via extra-contractual arrangements.
• Monitors care plan effectiveness and updates plans as needed.
• Collaborates with Medical Directors and Physician Advisors on treatment plans.
