Job summary

Elevance Health is seeking a Nurse Case Manager I to perform comprehensive care management for members with complex needs. The role offers flexibility and autonomy in a virtual work environment, with in-person training sessions and a full-time schedule in a US-based setting.

Responsibilities include assessing needs, developing and coordinating care plans, and interfacing with Medical Directors. Requires RN license and 3+ years of clinical experience; multi-state licensure is required if serving

Qualifications

• BA/BS in a health-related field is required.
• Minimum of 3 years of clinical experience is required.
• Current, unrestricted RN license in applicable state is required.
• Multi-state licensure is required if providing services in multiple states.
• Certification as a Case Manager is preferred.
• Experience with home health or Med-Surg is preferred.

Responsibilities

• Ensure member access to services appropriate to their health needs.
• Conduct assessments to identify individual needs and develop care management plans.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate the effectiveness of care management plans and modify as necessary.
• Interface with Medical Directors and Physician Advisors on care management treatment plans.
• Negotiate rates of reimbursement, as applicable.
• Assist in problem solving with providers, claims, or service issues.
