Job summary

Elevance Health is seeking a Nurse Case Manager I to support members with complex and chronic health needs. The role can be performed virtually full-time with required in-person training sessions, promoting flexibility and autonomy.

You will assess, develop, implement, and coordinate care plans, facilitating authorizations and referrals while collaborating with medical directors to optimize care across the continuum. Strong communication and nursing license are required.

Qualifications

• BA/BS in a health-related field or equivalent experience.
• Minimum 3 years of clinical experience.
• Current, unrestricted RN license in applicable state.
• Multi-state licensure if serving multiple states.

Responsibilities

• Assess, develop, implement, coordinate, monitor, and evaluate care plans for members with complex needs.
• Perform care management activities telephonically or on-site (e.g., discharge planning).
• Facilitate authorizations/referrals within benefits or through extra-contractual arrangements.
• Coordinate internal and external resources to meet identified needs.
• Monitor outcomes and adjust plans as needed.
• Collaborate with Medical Directors and Physician Advisors on treatment plans.
• Support problem-solving with providers, claims, or services.
