Job summary

Catapult Health seeks a Nurse Practitioner with multi-state licensing to conduct remote preventive assessments via telehealth while coordinating from our Dallas, TX office. You will review diabetes, heart attack, and stroke risk, screen for depression, and craft individualized action plans.

Strong NP certification (FNP/AGNP/ANP) and 3+ state licenses are required, with 80+ hours/month availability. Experience in primary care or internal medicine, Spanish language skills a plus, and proficiency

Qualifications

Current TX RN license and compact TX APRN license.
Licensed in at least 3 additional states as NP and willingness to license in 3–5 more.
National certification as FNP, AGNP, or ANP required.
Availability to work minimum 80 hours per month across Mon–Sun shifts.
Minimum 1 year NP practice in Primary Care or Internal Medicine.
Strong computer, communication, and interpersonal skills.
Bilingual Spanish is a strong plus.

Responsibilities

Conduct private preventive assessments via Catapult video platform.
Educate patients on diabetes, heart attack, stroke risk and prevention.
Document consultations in Catapult's EMR.
Create individualized action plans per patient results.
Assess depression risk and refer to resources as appropriate.
Refer patients to employee carrier resources based on results.
Engage patients to own their future health plans; provide guideline-based counseling.
