Job summary

Twin Health in the United States seeks an Advanced Practice Provider to deliver treatment, management, and guidance for diabetes reversal and metabolic health across multiple states via remote care.

You will work with a health coach, RN, and provider care management model, conduct health assessments, review labs, and prescribe medications per Twin Health policy, while documenting in Twin's clinical platform.

Qualifications

• ARNP license in CA or ability to obtain multi-state licensure.
• Bilingual English and Spanish preferred.
• 3–5 years as an advanced practice provider in family, internal medicine, endocrinology, obesity medicine, or chronic care management.

Responsibilities

• Engage with a collaborative team of healthcare professionals.
• Conduct health assessments including lab review and medical history.
• Assess symptoms and provide appropriate treatment in collaboration with the member's primary care provider.
• Deliver care via telemedicine and document in Twin Health’s clinical platform.
• Manage high-risk patients across multiple states with care coordination.
• Learn and apply the Twin Model of Care to support diabetes reversal and other conditions.
