Job summary

Twin Health, Inc. is seeking an experienced Advanced Practice Provider to support diabetes reversal and metabolic health across multiple states through remote care management. You will deliver care via telemedicine with a clinical care team, use Twin’s platform, and ensure high-quality patient experiences.

The role requires CA licensure with multi-state capabilities, strong communication, and a patient-centered approach. Remote work flexibility and a collaborative culture are emphasized.

Qualifications

• Physician Assistant CA state license; multi-state licensure preferred.
• Bilingual English and Spanish.
• 3–5 years as an advanced practice provider.
• Experience with underserved populations.
• Remote patient management across states.
• 5+ licenses with willingness to obtain more.
• Collaborative with physicians and health coaches.
• Board certification in family or internal medicine.
• Proven advanced practice provider experience.
• Strong medical knowledge.
• Virtual patient consultations skills.
• Excellent analytical and problem-solving abilities.
• Exceptional communication skills.
• Patient, compassionate disposition.
• Detail-oriented.
• Experience using data/tech to guide care decisions.

Responsibilities

• Engage with healthcare team including health coaches and physicians.
• Conduct health assessments and review labs/history.
• Assess symptoms and treat, collaborating with primary care.
• Deliver care via telemedicine and document in clinical platform.
• Manage high-risk patients and consider social determinants.
• Learn Twin Model of Care to support diabetes reversal.
• Understand SLAs and align clinical/financial outcomes.
• Collaborate with medical team to ensure quality patient care.
• Educate patients to promote healthy habits.
• Discuss medical history, symptoms, medications.
• Prescribe appropriate medications per Twin policy.
• Maintain accurate patient records.
• Execute additional duties as assigned.
