Altea Healthcare is seeking a Certified Nurse Practitioner (NP) to join its post-acute care team in a remote, part-time capacity. The role supports national CCM visits with flexible weekday coverage from 7:00 AM to 7:00 PM or 7:00 PM to 7:00 AM, rotation-based with other providers.

A Michigan NP license and DEA registration are required, with training and mentorship available for all experience levels. The ideal candidate brings experience in geriatrics or internal medicine and a patient-first

Qualifications

• Active Certified Nurse Practitioner license in Michigan is required before start.
• Current DEA registration or willingness to obtain prior to start date.
• All experience levels welcome; training and mentorship available.
• Passion for geriatrics, internal medicine, or primary care.
• Strong team player with excellent communication skills.
• Self-motivated with a patient-first approach.

Responsibilities

• Examine patients' medical records and update documentation.
• Order and interpret diagnostic tests (labs, imaging).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Oversee patients’ overall health and promote good health practices.
