Medix is seeking a clinically focused pharmacist for a fully remote role focused on medication management and patient care programs. You will engage with patients telephonically, educate on therapies and chronic disease management, and support care teams in optimizing medication outcomes.

The position emphasizes documentation, collaboration with healthcare providers, and participation in quality improvement initiatives, with a strong focus on adherence and appropriate therapy monitoring.

Formación

• PharmD and active pharmacist license required.

Responsabilidades

• Conduct telephonic medication reviews and patient consultations to optimize medication outcomes.
• Provide education on therapies and chronic disease management.
• Complete reconciliations and assess opportunities to improve therapeutic outcomes.
• Evaluate patient data to identify medication-related concerns and interventions.
• Document encounters, recommendations and outcomes accurately.
• Collaborate with physicians, pharmacies, care teams and patients to address issues and enable timely follow-up.
• Recommend evidence-based interventions to improve medication utilization and adherence.
• Manage a consistent volume of patient interactions while meeting quality and productivity standards.
