Job summary

Qualifications

• Degree or equivalent and typically requires 2+ years of relevant experience.
• Licensed Pharmacist in Delaware and able to get licensed in Illinois quickly.

Responsibilities

• Conduct initial patient evaluations and work-ups to optimize drug therapy.
• Coordinate with physician staff to implement patient care plans for safe delivery of drug therapy.
• Maintain thorough documentation in the patient clinical chart and communicate with other healthcare professionals.
• Establish therapeutic objectives and monitor outcomes, detect adverse reactions and complications.
• Provide counseling and answer questions from patients and family about drug therapy.
• Instruct patients on effects of food and alcohol and warn of side effects.
• Provide drug information and assistance to medical staff; assist with medication access.
• Prepare and dispense medication orders; perform QA checks on orders and verify accuracy.
