THIS IS NOT A REMOTE POSITION. WILL WORK ONSITE AT PALMETTO COMMERCE PARKWAY

HOURS: Mostly 6:00am - 2:30pm OR 1:00pm - 9:30pm

Primary Function/General Purpose of Position The Care at Home Specialist supports the day-to-day operations of the Care at Home programs including Remote Patient Management (RPM) and Hospital at Home. The Care at Home Specialist is responsible for the program development, logistics, inventory, and servicing of telemonitoring, emergency response equipment, and additional supplies & equipment necessary for all programs. The Remote Monitoring Equipment Specialist will be the technology subject matter expert to ensure an optimal patient experience. The Care at Home Specialist will coordinate and communicate with vendors, customer service departments, and technicians as well as the Remote Patient Management Program and Hospital at Home teams as needed. The Care at Home Specialist is responsible for inventory control that assures all equipment is retrieved in a timely manner when services are discontinued as well as cleaning and inspecting the equipment before redistribution. The Care at Home Specialist is an advocate of the programs and will support continuous improvement that assures success and sustainability for the community we serve.

Essential Job Functions

  • Position has overall responsibility for the day-to-day oversight of monitoring technologies, equipment, and supplies including logistics, operations, cleaning and maintenance, ordering and inventorying, as well as coordination with appropriate vendor or care team member.
  • Position is responsible for supporting the growth of the program to include education/training of clinical staff, community resources, potential referral sources, and procurement needs.
  • Responsible for direct marketing initiatives, ongoing community presentations, regular agency in-services, and other marketing items as well as for any special grants or projects.
  • Position is responsible for oversight of daily financial transactions regarding subscribers including billing and collections.
  • Tracking of expenses and contracts for optimal expense management.
  • Monitors measurements for trends and recommends program adjustments/redesign to better serve the desired population.
  • Performs initial contact, set-up, and delivery with installation of telemonitoring and/or emergency response equipment in the patient’s residence.
  • Provides education and training for the patient/caregiver/care team on the use of equipment.
  • Performs routine maintenance, troubleshooting, inventory, and inspection of the assigned equipment.
  • Coordinates with the Telemonitoring nurse, Lifeline group, and clinicians on equipment issues, patient identified needs, and scheduling.

Licensing/Certification

Education

  • High school diploma or equivalent (required)
  • Associate degree or technical certification (preferred)

Work Experience

  • One year of experience in a home health environment, geriatrics, social work, or community health organization or one year of experience with equipment installation and management (required)

Working Conditions/Physical Requirements

Frequent bending, stooping, standing, stretching, walking and sitting. Requires full range of body motion, including, handling and lifting equipment, manual and finger dexterity, eye-hand coordination and safe driving skills. Occasionally lifts and carries items weighing up to 50 pounds. Requires frequent talking in person and on the telephone. Vision correctable to within normal limits. Hearing required (hearing aid permitted). Requires fine motor coordination, operation of motor vehicle (or available transportation), operation of medical and office equipment or machinery. Other: access to a variety of clinical locations. Requires traveling locally. Exposure to environmental factors (weather, unsafe home environments), chemicals, solvents, bactericidal agents, body fluids, and animals.

Skills

Hard/Tech/Clinical Skills :

  • Proficient use of a computer, including but not limited to spreadsheets.
  • Knowledge of equipment troubleshooting and basis installation and set-up.
  • Must have the ability to use standard office equipment such as telephones, fax machines, computer terminals, etc.

Soft/Interpersonal Skills

  • Must be able to work efficiently under stressful conditions and time constraints.
  • Demonstrated skills in providing education to others.
  • Demonstrates appropriate verbal and written communication skills.
