Job summary

Brown University Health is seeking a Pharmacy Medication History Technician Specialist to compile home medication histories and allergy data for hospital admissions. The role involves interviewing patients and families, contacting pharmacies and facilities, and documenting in the EMR with high precision.

You will work with pharmacists and the Medication History Pharmacy Technician Supervisor in a remote-supported model while maintaining excellent customer service and collaboration with care

Qualifications

• Pharmacy Technician license/ certification current in Rhode Island.
• 1–2 years of outpatient pharmacy experience preferred.
• Strong customer service and communication skills.
• Proficient with Microsoft Office and email/calendar apps.
• Comfortable using multiple pharmacy and system-wide applications.

Responsibilities

• Assemble home medication histories including drug, dose, form, frequency and route.
• Document allergies accurately in EMR and update as needed.
• Interview patients and families to obtain current medication data from sources.
• Communicate discrepancies to pharmacist and ensure follow-up closure.
• Ensure timely, accurate documentation and high-quality customer service.
