Job summary

Health Dimensions Group is seeking an experienced MDS Consultant – RN to support skilled nursing communities across Minnesota. You will provide remote and on-site assistance, strengthen reimbursement and documentation, and guide teams through the RAI, PDPM, and related processes.

The role requires current MN RN licensure, formal MDS experience in SNF/long-term care, and the ability to educate and collaborate with interdisciplinary teams. Travel within Minnesota is expected.

Qualifications

• Must have current Minnesota RN license and formal MDS experience in skilled nursing/long-term care.
• Strong knowledge of MDS, RAI, and skilled nursing reimbursement processes.
• Familiarity with PDPM and regulatory standards for SNFs.

Responsibilities

• Provide on-site MDS consulting and hands-on support to MN communities.
• Complete and oversee accurate MDS assessments per federal/state requirements.
• Support MDS coverage during vacancies, leaves, or periods of need.
• Review MDS processes and documentation for accuracy and compliance.
• Assist with reimbursement through accurate assessment, documentation, and coding.
