Provide on‑site MDS consulting and hands‑on support to skilled nursing communities throughout Minnesota
Complete and/or oversee accurate and timely MDS assessments in accordance with applicable federal and state requirements
Support communities with MDS coverage during vacancies, transitions, leaves of absence, or other periods of operational need
Review MDS processes, clinical documentation, and supporting records for accuracy, completeness, and compliance
Support appropriate reimbursement through accurate resident assessment, documentation, and coding practices
Provide guidance related to the RAI process, PDPM, care planning, and reimbursement
Collaborate with nursing, therapy, social services, dietary, and other members of the interdisciplinary team to ensure accurate resident assessments
Identify documentation or process gaps and provide practical recommendations for improvement
Educate and mentor MDS Coordinators, nursing leaders, and interdisciplinary team members on MDS requirements and best practices
Assist communities in developing effective systems and processes for assessment scheduling, completion, review, and submission
Support audit preparation, survey readiness, and regulatory compliance related to MDS and resident assessment requirements
Monitor key MDS and reimbursement indicators and identify opportunities for improved accuracy and performance
Communicate findings, risks, and recommendations clearly to community and organizational leadership
Help establish sustainable processes that community teams can maintain following the consulting engagement
Current Minnesota Registered Nurse (RN) license in good standing
Formal MDS experience in a skilled nursing/long-term care setting required
Strong working knowledge of the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), and skilled nursing reimbursement processes
Knowledge and practical experience with PDPM
Experience completing, reviewing, and managing MDS assessments and associated documentation
Strong understanding of skilled nursing clinical documentation and interdisciplinary care planning
Knowledge of CMS requirements and regulatory standards applicable to skilled nursing facilities
Ability to review documentation, identify discrepancies or opportunities, and provide clear recommendations
Strong analytical, organizational, and problem‑solving skills
Ability to educate, coach, and build relationships with nursing and interdisciplinary teams
Ability to work independently and quickly adapt to different communities, systems, and team environments
Flexibility and willingness to travel throughout Minnesota based on client and community needs
Previous MDS consulting, regional MDS, multi‑site, or interim experience is strongly preferred
RAC‑CT or other relevant MDS certification is preferred but not required
