Job summary

Qualifications
- Current and valid New Jersey RN license.
- Prior MDS Coordinator experience is required.
- Strong knowledge of MDS 3.0, RAI guidelines, PDPM, Medicare, and skilled nursing regulations.
- Experience with PointClickCare preferred.
- Strong organizational skills and attention to detail.
- Ability to manage assessment deadlines and competing priorities.
Responsibilities
- Coordinate and complete MDS assessments accurately and within required timeframes
- Develop and maintain assessment schedules for Medicare, Medicaid, and OBRA residents
- Ensure compliance with federal and New Jersey regulations
- Lead interdisciplinary care-plan meetings and ensure care plans reflect residents’ needs
- Review clinical documentation for accuracy, completeness, and reimbursement support
- Monitor Medicare eligibility, skilled coverage, and assessment requirements
- Apply current PDPM guidelines and identify appropriate clinical categories
- Collaborate with nursing, therapy, social services, dietary, and other departments
- Educate clinical staff on documentation requirements and MDS-related processes
- Monitor quality measures and help identify opportunities for improvement
- Participate in audits, surveys, and utilization-review activities
