Job description

Join to apply for the Remote MDS Coordinator role at Recover-Care Healthcare.

As a MDS Coordinator, you will assist our Director of Nursing and the RN Assessment Coordinator in ensuring documentation compliance with Federal, State, and Certification guidelines. You will also coordinate the RAI process, ensuring the timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.

GENERAL DUTIES
  • Assist in ensuring adherence to Federal and State regulations and certification requirements.
  • Participate in regulatory or certification survey processes and help correct deficiencies.
  • Report trends from audits to the Quality Assurance Committee.
  • Ensure completion of the RAI process from MDS to interdisciplinary care plan.
  • Monitor RAI process tracking, discharge/reentry, and Medicaid forms through PointClickCare.
  • Follow up with staff to ensure documentation standards are met.
  • Complete patient assessments, data collection, and staff interviews as needed, following the MDS User’s Manual.
Qualifications
  • Graduate of an approved RN program and licensed in the state of practice.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
  • Understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set; experience with care planning.
