Qualifications

• Must reside in Illinois.
• Active RN or SW license.
• 5+ years professional experience.
• 2+ years management or supervisory experience.
• Proficiency in analyzing and interpreting data trends.
• Progressive operational leadership experience.
• Strong communication, analytical and teamwork skills.
• Knowledge of Medicaid/Medicare and long-term care policies.
• Proficient in MS Windows suite and MIS software.

Responsibilities

• Lead assessment and evaluation of members' needs to achieve optimal wellness.
• Guide members and families to appropriate care resources.
• Collaborate with care management extenders and partner organizations.
• Monitor program performance and drive improvements.
• Ensure compliance with federal, state, and contract requirements.
• Develop policies to support high-quality service delivery.
• Support training for care management extenders.
• Coordinate with operations for cost-effective, quality care delivery.
