Qualifications

• Active, unrestricted LPN license.
• One year or more years of experience working as an LPN.
• Experience with Electronic Health Record systems for documentation, data management, and workflow optimization.
• Demonstrated ability to solve complex problems and encourage collaborative problem-solving.
• Proven ability to work independently while contributing to team success.

Responsibilities

• Oversee medication therapy, strategy on med reviews, and prescription drug optimization for patients on multiple meds.
• Promote health awareness with Rx Education and quarterly campaigns.
• Place outbound calls to confirm patient adherence and provide counseling.
• Adhere to professional standards, regulations, and documented procedures.
• Make decisions regarding own work priorities and follow direction.
