Qualifications

Responsibilities

• Perform Chronic Care Management tasks under RN supervision and develop care plans.

• Document findings and maintain HIPAA-compliant records in EHR.
• Communicate clearly with patients and coordinate care with the healthcare team.
• Schedule management and patient education to improve outcomes.
