Job summary

Qualifications

• Current, unrestricted State of Arizona LPN license.
• Two years of experience in a family practice or pediatric setting.
• Ability to travel to all contracted sites, with possible overnight stays.
• Ability to work independently or as part of an interdisciplinary team.
• Excellent verbal and written communication.
• Strong collaboration and confidentiality.
• Knowledge of HEDIS/AHCCCS/CMS standards and reporting requirements.

Responsibilities

• Triage incoming referrals and assign cases.
• Oversee case resolution to ensure regulatory compliance.
• Abstract medical records and analyze data for QA purposes.
• Prepare Corrective Action Plans with Credentials Committee as directed.
• Report audit findings and communicate with providers and Credentialing Department.
• Collaborate with QM team to support data-driven improvements.
