Adaptable Human Solutions Clinic is seeking a Licensed Professional Clinical Counselor (LPCC) for a full-time, primarily remote role with an outpatient office in San Jose, CA. You will perform comprehensive assessments, develop therapeutic plans, and deliver high-quality mental health counseling to diverse clients.

Qualifikationen

• Master's or Doctoral degree in Counseling or Psychology required.
• Active and unrestricted LPCC licensure in California.
• Minimum of two years counseling experience in a clinical setting.
• Preference for experience in family therapy, couples therapy, substance misuse, or trauma care.
• Strong grasp of legal and ethical standards in mental health.
• Excellent interpersonal and communication skills for diverse clients.
• Ability to work effectively within a healthcare team.
• Proficiency in using EMR software and related technologies.

Aufgaben

• Conduct thorough assessments to understand each patient’s mental health needs.
• Collaborate with supervisors, direct care staff, families, and other stakeholders.
• Draft and share a tentative schedule with supervisors for appointments and visits.
• Coordinate home visits and data collection while following observation protocols.
• Implement and monitor the therapeutic program plan for tailored care.
• Participate in Interdisciplinary Team Meetings and care discussions.
• Document monthly and quarterly progress notes reflecting client progress.
• Organize and conduct staff training on clinical plans and documentation.
• Provide ongoing follow-up interviews and additional home visits as needed.
• Maintain accessible records for easy retrieval of patient information.
• Serve as a liaison with the public and mental health community for program goals.
• Contribute to creating training materials to enhance program methods.
