Qualifications

• MSW with LCSW licensure required.
• 2–4 years social work experience within the last 5 years, preferably in managed care.

Responsibilities

• Provide telephonic psychosocial assessments and interventions.
• Educate members, family and caregivers on adaptation to diagnosis, illness, and treatment.
• Address barriers to social determinants of health and connect to community resources.
• Coordinate with hospitals, SNFs, home health, hospice, and other agencies to advocate for ongoing care.
