Job summary

RIPIN in Rhode Island is seeking a Community Health Worker to support hospital-to-home transitions. You will provide options counseling, resources, and referrals to patients and families, focusing on post-discharge care and social determinants of health.

As a peer mentor, you will educate patients about Medicaid/Medicare benefits, assist with enrollment applications, and coordinate with care teams to establish timely supports in home and community settings.

Qualifications

• High School diploma or GED is required.
• CHW certification preferred; must earn within 18 months of hire.
• Experience navigating Rhode Island health systems and service providers.
• Strong written and oral communication skills.

Responsibilities

• Assist patients and families with post-discharge care options.
• Educate on Medicaid/Medicare benefits, including enrollment and cost sharing.
• Assist consumers with enrollment or benefit applications; refer to services as needed.
• Collaborate with hospitals, plans, and providers to coordinate post-discharge supports.
• Maintain timely, accurate records and reports; participate in training and professional development.
• Engage with consumers to support independence and case closure with follow-up.
