Job summary

Home Care Providers of Texas is seeking compassionate, full-time Registered Nurses (RN) and clinicians to provide in-home care in the San Antonio, TX area. You will work with a team that supports your schedule and independence, delivering high-quality nursing care in patients’ homes.

The role emphasizes modern tools, 24/7 staff support, paid training, and a focus on Honor, Kindness and Patience. Experience in geriatric home health care and a valid TX license are required.

Qualifications

Active RN license valid in TX or compact state.
Active CPR certification.
Experience in geriatric home health care.
Self-starter with strong organizational skills.
Compassionate care approach.
Valid driver’s license.

Responsibilities

Deliver skilled nursing care in patients' homes according to physician orders and care plans.
Collaborate with the care team to develop and update personalized plans of care.
Administer nursing treatments and manage clinical activities.
Monitor patient progress and communicate findings to the team, patient, family, and case manager.
Complete additional duties as needed.
