Responsibilities

• Deliver Skilled Nursing Care in the patient’s home per physician orders and care plan.
• Collaborate with the care team to develop and update personalized care plans.
• Administer nursing treatments and manage clinical activities to support health outcomes.
• Monitor patient progress and communicate findings to the care team, patient, family, and case manager.
• Perform additional duties as needed.
