Job summary

Prudential Healthcare is seeking a qualified Home Health PTA in California to support patients under a licensed PT. You will lead prescribed exercises, monitor progress, and educate patients and caregivers while maintaining accurate documentation in the EMR.

The role offers flexible scheduling and travel within designated areas. A CA PTA license and at least 1 year of home health experience are preferred. Strong communication and reliable transportation are essential.

Qualifications

• Licensed Physical Therapy Assistant (PTA) in CA.
• Minimum of 1 year PTA experience (home health preferred).
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver’s license.
• Knowledge of Medicare/Medicaid guidelines a plus.
• EMR experience preferred.

Responsibilities

• Assist patients with prescribed physical therapy exercises and treatments.
• Monitor patients' progress and provide feedback to the supervising PT.
• Educate patients and caregivers on exercises, techniques, and safety precautions.
• Maintain accurate documentation of patient care and progress.
• Communicate with the healthcare team to ensure coordinated care.
• Ensure proper use and maintenance of equipment and supplies.
• Assist with patient evaluations and goal setting as directed by the PT.
• Participate in on-call rotation (if required).
