Job summary

Northern Rivers Family of Services is seeking a Home Based Crisis Intervention Specialist to support families and youth in crisis with in-home services, using behavior management and de-escalation techniques.

The role features a hybrid schedule, on-call crisis support, weekly meetings, and opportunities to work remotely for administration; a NYS driver’s license is required and licensure is preferred for several clinical licenses.

Qualifications

• Bachelor’s degree or Master’s Degree with 1-year relevant experience in community-based health or case management.
• LMSW, LCSW, LMHC, LMFT or LCAT licensure preferred.

Responsibilities

• Respond to the needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling.
• Promote safety of the child, family, and community in the delivery of crisis intervention services.
• Tailor services and flexibly schedule sessions based on family member’s needs.
• Conduct comprehensive assessment of family strengths, values, skills, needs, and collaborate in goal setting.
• Use a variety of teaching methods to help family members acquire, maintain and generalize skills.
• Advocate for and provide concrete services and supports and teach family members to meet their concrete needs.
• Collaborate with community resources and systems that impact families while teaching self-advocacy.
• Prepare family for transition and service closure.
• Provide consultation to intra-agency and community providers and maintain liaisons with external and internal partners.
• Fulfill all program-reporting requirements.
• Participate in the Continuous Quality Improvement process and provide leadership in data collection and recommendations.

Education
