Job summary

UnitedHealth Group is seeking a Registered Nurse to support community health initiatives in the McAllen, TX area with flexible remote work options. The position requires an active Texas RN license, 2+ years of experience in healthcare, and the ability to travel within the territory.

Qualifications

Current Texas RN license.
2+ years of experience in a community health or healthcare setting.
Intermediate level of proficiency with Microsoft Office applications.
Access to reliable transportation and valid US driver's license with the ability to travel within assigned territory.
Preferred: Medicaid, LTSS, or long-term care experience; HCBS and integrated care knowledge; bilingual in English and Spanish.

Responsibilities

Assess, plan, and implement person-centered strategies.
Connect members with financial, psychosocial, community, and state resources.
Coordinate care across the continuum and serve as a key point of contact.
Advocate for members and families to ensure their needs and preferences are heard and supported.
Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
