Qualifications

• Experience as an RN Medical Case Manager is ideal or a clinical background in orthopedics, neurology, or rehabilitation is preferred.
• A valid driver's license and reliable transportation is required.
• Ability to travel to assigned locations is required.

Responsibilities

• Provide in-person and telephonic Medical Case Management to individuals.
• Discuss treatment plans with physicians utilizing medical and nursing knowledge.
• Conduct initial evaluations through home visits and facilitate patient care.
