Qualifications

• Current unrestricted Texas RN license.
• 2+ years of experience in a community health or healthcare setting.
• Intermediate level of proficiency with Microsoft Office applications.
• Reliable transportation, a valid driver, and ability to travel within assigned territory.

Responsibilities

• Assess, plan, and implement person-centered strategies.
• Connect members with financial, psychosocial, community, and state resources.
• Coordinate care across the continuum and serve as a key point of contact.
• Advocate for members and families to ensure needs and preferences are heard.
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
