Job description

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in the environment that patients feel most comfortable and are most receptive to, including home, nursing facilities, and pop-up clinics.

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to all patient levels. Some programs may focus on specific populations, such as adults, geriatrics, pediatrics, or women's health.

Responsibilities include performing comprehensive medical assessments, ordering tests and procedures for diagnostics, formulating treatment plans, obtaining specialist consultations as needed, and documenting all activities appropriately.

Job Duties
  1. Provide general medical care and care coordination for diverse patient groups including adults, women’s health, pediatrics, and geriatrics.
  2. Perform comprehensive evaluations, including history and physical exams, to identify gaps in care and conduct preventative assessments.
  3. Address both chronic and acute primary care complaints, assessing medical urgency.
  4. Establish and document accurate medical diagnoses.
  5. Seek specialty consultations when appropriate.
  6. Order and perform relevant diagnostic tests, using resources judiciously.
  7. Recognize when patient needs exceed scope of practice and require physician oversight.
  8. Create and implement medical care plans.
  9. Schedule telehealth or in-person patient appointments as appropriate.
  10. Coordinate post-discharge care to reduce hospital readmissions and ER visits.
  11. Conduct face-to-face visits in various settings, including homes, nursing facilities, and public locations.
  12. Perform synchronous video visits via Telehealth platforms as needed.
  13. Order targeted laboratory tests for specific populations.
  14. Participate in on-call coverage for urgent lab and pharmacy inquiries, developing appropriate care plans.
  15. Engage in community-based Pop Up Clinics to build relationships and address healthcare gaps.
  16. Travel up to 120 miles daily within the assigned region; extended mileage days may occur.
  17. Maintain cross-state licensure based on business needs.
  18. Collaborate with fellow nurse practitioners to develop best practices.
  19. Participate actively in regional meetings.
  20. Prescribe medications and perform procedures as appropriate.
  21. Ensure timely documentation in electronic medical records.
  22. Occasionally, walk flights of stairs while carrying up to 50 lbs. of equipment.
Job Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience / Skills: Advanced computer skills; proficient in Word, Excel, and EMR systems.

Required Licenses and Certifications:

  • Active, unrestricted national certification from the American Academy of Nurse Practitioners or ANCC.
  • Current state-issued license as a Family Nurse Practitioner.
  • Current BLS certification for Healthcare Professionals.
Preferred Qualifications

Preferred Education and Experience:

  • 3-5 years as a Registered Nurse or Nurse Practitioner, preferably in home, community, or public health settings.
  • Experience managing chronic conditions in home health.
  • Experience serving underserved populations with socioeconomic barriers.
  • Fluency in additional languages is a plus.
  • Skills in immunization and point-of-care testing.

Pay Range: $72,370.82 - $156,803.45 annually
