Job summary

Boost Home Healthcare is seeking a compassionate Registered Nurse to join as a Home Health Case Manager. You will coordinate care across the interdisciplinary team, conduct OASIS assessments, and ensure timely services in patients’ homes with a focus on independence and quality care.

You should have an active RN license, at least 1 year in home health or case management, and comfort with EHR systems. Flexible scheduling and supportive team culture are offered.

Qualifications

• RN license active in good standing
• Minimum 1 year experience in home health or case management
• Proficiency with EHR systems and care coordination
• Reliable transportation and valid driver’s license

Responsibilities

• Manage a caseload of home health patients and coordinate care
• Complete OASIS assessments and develop individualized care plans
• Collaborate with physicians, nurses, therapists, social workers, and aides
• Monitor patient progress and update care plans accordingly
• Maintain accurate clinical documentation per Medicare guidelines
• Educate patients and families to support independence and wellness
