UnitedHealth Group is seeking a Director of Population Health Care Transformation to lead ambulatory clinical programs, care management, and value-based initiatives across the enterprise. The role partners with physicians, operations, and payers to drive strategic planning, performance, and regulatory compliance while delivering measurable improvements in quality and cost of care.

The position supports remote work within the U.S.

Qualifications

• Current, active, unrestricted Registered Nurse (RN) License in the U.S.
• 8+ years of progressive healthcare leadership experience
• 5+ years in ambulatory clinical operations and population health or value-based care leadership
• Experience leading multidisciplinary teams and partnering with physicians and payers
• Able/willing to work Mountain Time Zone business hours
• Willing to travel ~25% of the time, primarily to the client site in Boulder, CO

Responsibilities

• Direct oversight of ambulatory care management and other ambulatory programs
• Supervise department managers and ensure efficient daily operations
• Develop and execute ambulatory population health strategies for value-based contracts
• Ensure programs align with performance measures and priorities
• Oversee cross-organization collaboration to align priorities and outcomes
• Design and optimize ambulatory clinical programs and workflows
• Lead implementation of new clinical initiatives and technology solutions
• Drive continuous improvement across ambulatory services
• Serve as the clinical operational lead for payer-sponsored programs
• Collaborate with physician leadership, finance, contracting, and quality to maximize performance
• Accountable for performance across readmissions, ED use, HEDIS/STAR, total cost of care, adherence, and engagement
• Establish standardized workflows and metrics; mentor managers and leaders
